I hereby apply for enrolment as a member of the RCCG Staff Workers Fund and promised to abide with the rules and regulations in Bye-Law and as amended from time to time. Being a member of the above named workers fund hereby declare that the person's name below shall be my next of kin. The terms of the bye law of the above mentioned workers fund namely the person persons whom in the event death all the sum due to me by the society shall be transferred.
